Our Mission

Bridging the Gap Between Community and Care

Outer Cape Health Services (OCHS) provides comprehensive primary care and behavioral health services to some of the most geographically isolated communities in Massachusetts. Despite strong community trust and high‑quality care delivery, structural barriers—including workforce shortages, transportation challenges, and seasonal population surges—limit timely access to care.

This project will establish the infrastructure needed to deliver care more flexibly, efficiently, and equitably through telehealth.

The Challenge

Addressing Barriers to Care in a
Rural Coastal Region

📍

Geographic Isolation

Long travel distances for routine care create significant access barriers, particularly during winter months when transportation options are limited.

🩺

Workforce Shortages

Difficulty recruiting and retaining clinicians in a high‑cost coastal region strains capacity and continuity of care throughout the year.

🌊

Seasonal Surges

Summer population influx dramatically strains system capacity, creating bottlenecks in scheduling and clinical bandwidth.

👵

Aging Population

A large and growing older adult population has elevated chronic care and behavioral health needs requiring specialized, ongoing support.

🚐

Transportation Limitations

Limited public transit and financial barriers prevent older adults and low‑income residents from reliably accessing needed appointments.

Telehealth offers a scalable solution—but only with the appropriate infrastructure in place.

The Solution

Telehealth Capacity Expansion Project

🖥️

Telehealth‑Enabled Clinical Spaces

Fully equipped telehealth rooms at all OCHS sites, designed for high‑quality remote encounters with full technical and clinical support.

📡

Remote Patient Monitoring

Deployment of RPM technologies enabling continuous monitoring of chronic disease patients between visits, improving outcomes and reducing hospitalizations.

💬

Tele‑Behavioral Health

Expanded telepsychiatry and tele‑counseling services addressing the region's significant unmet behavioral health demand.

🎓

Workforce Training

Comprehensive training programs and hybrid workflow integration ensuring clinical staff are confident and efficient in telehealth delivery.

🏠

Home‑Based Telehealth Access

Beginning in 2027, OCHS will deploy secure, HIPAA‑compliant telehealth infrastructure—including patient devices and implementation support—to deliver care directly to patients’ homes and reduce access barriers related to geography, mobility, and transportation.

Outer Cape Health Telehealth Project
